Susan Colemer, MSN, AGPCNP-BC

Susan Colemer, MSN, AGPCNP-BC is a board-certified Adult-Gerontology Primary Care Nurse Practitioner who started her career in inpatient bedside nursing working with patients that had diverse comorbidities, orthopedics, and outpatient specialty clinic in a burn and wound clinic. After working as a nurse for several years, she attended Concordia University in Mequon, WI to obtain her Masters in Nursing with honors. Susan is an active member in the American Association of Nurse Practitioners and Sigma Theta Tau International Honor Society of Nursing. Throughout Susan’s career in healthcare, she has maintained numerous leadership roles through precepting new graduate nurses, as a unit council member and competency champion, and RISC Wound and Skin nurse for an inpatient unit.

Susan has always been motivated to learn and help others through the knowledge she has attained leading a healthy and happier life. A holistic approach to medicine has always been a passion of Susan’s and she believes in treating patients as a whole person, not just the disease or condition. Susan enjoys developing strong relationships and educating her patients to understand and develop a plan together that is attainable and sustainable. She is particularly passionate about preventative healthcare, hormone optimization, and wound care.

Born and raised in Madison, WI, Susan recently relocated to Oregon with her husband to join OnePeak Medical in Lake Oswego. In her free time, she enjoys yoga, singing, camping, and hiking with her husband and two dogs, Bailey (Siberian Husky) and Riley (Golden Retriever Lab Mix). When she’s not outside, you can find her relaxing on the couch or painting with her cat Neeko (Tabby mix) nearby.
